I got taken off Tramodol and Amitriptline. They were brilliant meds, I was told iv got Intollerence to all pain meds, I can only have paracetamol & they don't touch the pain.

What medications can help with fibromyalgia symptoms?

There are currently four FDA-approved medications for fibromyalgia, and many others are prescribed "off-label" β€” meaning doctors use them because they may help, even if not officially approved for fibromyalgia specifically.

Here's a breakdown of the main categories:

Antidepressants
- SNRIs like duloxetine (Cymbalta) and milnacipran (Savella) are FDA-approved and often used first, especially if fatigue or low mood are major symptoms
- Tricyclics like amitriptyline are used off-label and have a long history of helping with fibromyalgia
- SSRIs like fluoxetine (Prozac) or sertraline (Zoloft) may be tried when other options don't work

Anti-Seizure Medications
- Pregabalin (Lyrica) is FDA-approved and can help reduce pain and improve sleep
- Gabapentin (Neurontin) is used off-label and may also help with nerve-related symptoms Muscle Relaxants
- Cyclobenzaprine (Tonmya) recently received FDA approval in 2025 β€” it's a sublingual tablet (dissolves under the tongue) that works quickly with fewer side effects
- Others like carisoprodol (Soma) may help with stiffness but are only for short-term use

Sleep Medications
- Melatonin supplements or prescription sleep aids can help with sleep disruption
- Benzodiazepines (like Valium or Xanax) are sometimes used cautiously, but can cause dependence

Pain Relief
- Over-the-counter pain relievers may offer some relief
- Opioids are generally no longer recommended due to dependence risks

Every person responds differently to medications, so it's worth having an open conversation with your doctor about what combination might work best for your specific symptoms. Side effects like drowsiness are common with several of these, so it's important to discuss those risks too.
