Has anyone here had to fill out one of these and what was the outcome?
I'm assuming I need to be as detailed as possible and I am working at that.

@A MyFibroTeam Member, you are correct, the form they sent you is for you to fill out....not your doctor. There is a different form you can get from your lawyer to have your doctor fill out, I think it’s call a Residual Functional Capacity (RFC) form. That can be very helpful when they are deciding your case. Regarding the adult function report, take your time filling it out and be honest and consistent. Don’t be ashamed or embarrassed to say what you can and can’t do.....and most of all, don’t let it stress you out. Good luck 😊

Hi. When I received those papers I was told that they wer supposed to be filled out by my doctors. When I did answer questions on the forms the advice I was given was to be specific and not to overload with extra information. I know that is a subjective response. I finally had my hearing yesterday and I still have other forms to submit. It is overwhelming and kind of a pain but I'm hoping the judge will find in my favor. I hope this helps!

If it says for you e plain bow limited u are doing laundry dishes bending lifting showering the simplest easy day task the become Mount Everest to some of us. Good luck honey

I had to fill one out, and was very specific detailing what each activity did to me, for how long & severity of pain. Here are examples: Bending forward to wash dishes for a few minutes causes back & neck pain for an hour or more; must sit to rest;
Squatting down causes knee pain & difficulty getting up. A shower & dressing cause pain in back & chest, requiring I lie down for about 10 minutes until it passes. If I dont lie down or sit, it keeps hurting.
Anyway, those are just examples, but you have to write in honesty what things you experience... how doing this, causes that.
I had to keep revising my sentences as only so many characters are allowed to explain, so I had to explain clearly & briefly without losing the meaning of what I was trying to say. Make a list beforehand so you know what exactly you need to write. Also write how long you've been experiencing your symptoms. If you have any questions, I can try to answer. I did my SSDD Application online by myself & kept it honest & clear as possible. I went to all the appointments they requested me to, which included a doctor of their choosing, a Psychologist/psychiatrist and to a hospital for a couple x-rays. I gave a lot of medical history, dates, addresses of doctors, dates of surgeries, diagnosis date, etc... I know its a lot of work & concentration, but you'll get it done. Best of luck. God Bless. 😊

I had to go to tribunal every time... But I got pip at high rate and a car back in 2015 2016 they reassessed me and I lost everything but took it to tribunal and won back my pip but thus time only got low rate care and no mobility. Keep trying.
