I started a new job a couple of months ago working in a cafe. It's very busy and you don't get breaks. I am supposed to do 4hrs which is doable. But I'm getting pressure to stay behind more often etc which I'm finding too much, last time it was 5 and half hours. I would first get a note from a doctor then I would remind ur boss that when u got hired it was for only 4 hour shifts n due to ur condition that's all u can do. If they hired u know u could only work 4 hours they can't expect more

Breaks at work are compulsory not voluntary. Tell your boss that you are having your break, don't ask because that gives them the option to say no. As with staying over your contracted hours put your coat on and leave when your shift ends. You won't be any more thought of if you stay. As for thew 9 hr shift for stocktaking, you are allowed 90 mins for breaks. 2 x 15mins plus one hour or if better for you 3 x 30 mins breaks. There are organisations you can contact to find out your rights. I'd try looking for another job if I were you. A more understanding boss would go a long way to helping your stress levels.
