You need great doctor notes if you apply for SSDI. However, most physicians are very busy and some don't always document everything and all your symptoms- hence not creating a very accurate record. One website suggested we should look at these notes at least monthly. How do we go about doing that? Won't it offend the physician?

If they are good & ethical it won't offend them. before my clinics made records availble online I routinely requested my records.

I doctor gives me a copy of my notes every time I see him & I can also go online.
Just ask your doctors , usely you have to sign a form .
