I was thinking of applying for one...
My Dr put the dx of lumbar and cervical degenerative disc disease plus fibromyalgia. I didn't have to pay DMV, just turned in the paperwork and they mailed the placard. It's good for 2 yrs and they send me a new one every 2 yrs. I don't have to do anything else. Such a relief!
WOW! I've never heard of a PCP being uncooperative when you have a valid diagnosis. What a jerk! I would write to the local medical board or HMO group, if he's a member. It doesn't sound ethical to me, just sayin. I too wish you good luck with your new PCP.

I have the plaque and a tag for both vehicles. When I can’t walk it’s a blessing to have. This year in Oklahoma they have stopped charging for them. I couldn’t believe it. I recommend you get one. Hugs
Downhill, you get the form from your local dmv, fill it out and have a doctor, nurse, or other provider sign it then turn in to your dmv...
That's the best way to send a message to these rude and non helpful pcp. Good luck with your new pcp. You will find one who will work with you rather than against you.
